Form 1195 is an Identity Declaration form used in Australia to verify a person’s identity. It is required for certain applications, such as passport applications, where the applicant is unable to provide other acceptable forms of identification.
The form must be completed by a person who has known the applicant for at least one year and who can confirm their identity. The person completing the form must be an Australian citizen who is 18 years or older and who is not related to the applicant.
The form requires the following information to be provided:
- Applicant’s details, including full name, date of birth, and contact details
- Details of the person completing the form, including full name, date of birth, and contact details
- The nature of the relationship between the applicant and the person completing the form
- A declaration by the person completing the form that they have known the applicant for at least one year and that the information provided is true and correct.
The completed form must be signed by the person completing the form and witnessed by an authorized person, such as a Justice of the Peace, a Commissioner for Declarations, or a lawyer. The authorized person must also sign and date the form.
Once completed, the form should be submitted along with the relevant application to the relevant authority.
